Dr. Landmann evaluates lower-lid fat pads, skin quality, the eyelid-to-cheek transition, lid tone, eye-surface health, prior filler, swelling, discoloration, and any previous procedures. These findings determine whether surgery, a non-surgical option, or a combination should be discussed.

The surgical plan may include fat repositioning or removal, carefully selected skin treatment, and support for the lower eyelid. CO₂ resurfacing or another adjunct may be discussed when texture is part of the concern. The goal is a smoother, supported contour—not simply removing as much tissue as possible.

Lower-eyelid surgery is generally planned with twilight anesthesia in the appropriate accredited setting. Individual instructions cover preparation, transportation, swelling care, eye comfort, activity, and scheduled checks as the lower-lid contour settles.
